Main Activities of the Regional Support Center

(1) Creation of DVDs of "Fuku-roi Song" exercises and holding training sessions to promote these exercises.

The exercise program "10 Muscle Strength Training to Expand the Lives of the Elderly" used in the "Onishi Model," a resident-led long-term care prevention project, is being implemented in the Tone Numata area as Fukuro Song Gymnastics. The exercises are designed to help with daily activities such as walking, standing, sitting, straddling, climbing stairs, picking up objects from high places, and picking up dropped objects. It can be performed by participants alone without a leader, and can be enjoyed with friends in a familiar community.
The FUKURO Song Gymnastics training sessions can be viewed on YouTube. DVDs will be distributed to those who have answered the confirmation test after viewing the first and second training sessions.

(2) Holding workshops on oral flail prevention

Oral frail (deterioration of oral function), if left unchecked, can cause eating and swallowing difficulties, leading to a decline in physical function and the need for nursing care, so early detection and appropriate measures are important. We have held a total of three Oral Frail Prevention Workshops as training sessions to learn how to check for oral frail and improve oral function.

(iii) Organize events to prevent frailty in cooperation with government agencies and universities

The Corona disaster may have caused a decline in muscle mass and physical function among local residents. We planned and organized the event in the belief that understanding and analyzing these issues would contribute to the prevention of frailty. The first event was held at Katashina Village Hall, where a free physical fitness test was conducted with the cooperation of various people, including Dr. Shigeya Tanaka and students from Takasaki University of Health and Welfare, staff from Uchida Hospital, and supporters of care prevention in Katashina Village. About 30 local residents cooperated in the measurement. The measurement results were analyzed by Dr. Tanaka of Takasaki University of Health and Welfare, and feedback was provided to those in charge at Katashina Village Hall.
The second free physical fitness test was held at Kawaba Village's Kawaba Koryu Hall, with the cooperation of various people, including Dr. Shigeya Tanaka and students from Takasaki University of Health and Welfare, staff from Uchida Hospital, and care prevention supporters from Kawaba Village.
In addition to planning future frailty measures, we are also creating opportunities for cooperation among municipalities, universities, wide-area support centers, care prevention supporters, hospital facilities, etc., to build a system that enables cooperation on a daily basis.

(4) Newsletter publication

It introduces the activities of the wide-area support center and provides useful information for a wide range of people from children to the elderly.

(6) Loan of equipment for body composition meter and oral motor function tests (provided by the Prefectural Health and Longevity Division)

We have started lending out body composition analyzers (Tanita innerscan dual) and mouth movement function tests (Kenkuchi-kun Handy) for use in municipal care prevention projects and in support of local community outreach. A manual is also included so that professionals and employees of municipalities and comprehensive organizations can perform the test. 8) Skills training classes by professionals
 (formerly known as "on-the-job training") (knowledge and skills improvement through lectures and practical guidance by professionals)

We will provide consultation on rehabilitation and care methods for patients who are having difficulties in their daily lives, and help staff members improve their skills through lectures and practical guidance. This service is available free of charge for facilities that do not have rehabilitation staff or specialists on staff. Depending on the content of the consultation, specialists such as physical therapists, occupational therapists, speech therapists, dental hygienists, and music therapists can visit your workplace or provide non-contact consultation, lectures, and practical skills training via online (Zoom, etc.).
 Our services are utilized for consultations related to elderly facility users, skill improvement for staff, and consultations on problems related to children and how to relate to them. We accept a wide range of consultation topics and ages of the target audience. Tone Numata Community Qualification for Rehabilitation

(as of September 30, 2024)

The Japan Physical Therapists Association focuses on two measures, "community care conference" and "care prevention," to promote the "community comprehensive care system" and to develop human resources. The leader for promotion of community care conference is required to participate in the conference based on the purpose of the community care conference and be able to give advice that leads to support for self-reliance. In addition, the Care Prevention Promotion Leader is required to be able to plan and propose effective prevention programs in collaboration with other professions and residents, as well as assessments based on his/her expertise. In addition, in October 2020, a new qualification of manager for the promotion of measures against frailty was established in order to train personnel who can handle the integrated implementation of health projects and care prevention for the elderly. The manager is required to provide a wide range of support, including nutritional support and community development, management that links to medical and nursing care services as needed in cooperation with related organizations, and consultation support and training sessions related to frailty prevention.
The Japan Association of Occupational Therapists recommends the use of the "Management to Improve Lifestyle Activities" as a tool for training and practicing occupational therapists who can contribute to a "community-based comprehensive care system". Certified occupational therapists are able to provide advice at community care meetings and other forums on care management for individual subjects from the perspective of supporting their independence and enabling them to perform the activities of daily living that they desire.

2. the terminology on which the activity is based

1) Community Rehabilitation

Community rehabilitation refers to all activities in which all people, institutions, and organizations involved in daily life, including health, medical care, welfare, nursing care, and local residents, cooperate with each other from the standpoint of rehabilitation, so that children, adults, and elderly people with disabilities and their families can live in their own way and safely for the rest of their lives in their familiar places of residence. It refers to all activities conducted in cooperation from the standpoint of rehabilitation by all people, institutions, and organizations involved in daily life, including health, medical care, welfare, nursing care, and local residents.

(2) Flail and Flail Prevention

Frail is defined as a state of increased vulnerability to stress and poor health due to a decline in physiological reserve, and is "a state that is likely to lead to the need for nursing care or death. In other words, it is positioned as a state between the need for nursing care and normal health, and while the risk of needing nursing care in the near future is high, it is a reversible state with prospects for improvement.
Frail has three components: physical, cognitive, and social, each of which has a different frequency of onset, but approximately 10% of the population is classified as frail. Physical frailty is defined by the CHS criteria as weight loss, muscle weakness, fatigue, reduced ability to walk, and reduced activity. Cognitive frailty is the coexistence of physical and cognitive decline, and is determined using the criteria for physical frailty and a Clinical Dementia Rating (CDR) of 0.5. Social frailty focuses on social aspects and is defined by various factors such as satisfaction of social needs, social resources, general resources, and social activities.
Frail is expected to improve physical functions and even to break free from frailty through appropriate intervention.
