Information for hospitalized patients
Reservations for hospitalization
If you have an appointment for hospitalization, please come to the reception window on the 1st floor with your family member on the day and time of your appointment.
Please contact us in advance if you need to change, cancel, or delay your appointment.
Submission
Once you have been admitted to the hospital, please submit the following items to the receptionist
- hospital certificate
- health insurance beneficiary certificate
Please prepare the following only if you are certified.
- Certificate of Limit Amount Application and Standard Copayment Application
- welfare certificate
- specified disease beneficiary certificate
- Physical disability certificate
- Long-term care insurance certificate
Nurse Submission
When you are guided to the inpatient ward, please submit the following to the nurse
- Confirmation Form Concerning Personal Information
- Consent form for room charge difference (only for those who apply)
- Medications or instructions you have been taking
- Medical information form, nursing summary, discharge certificate (if you have one)
- Consent form and consent form (for those who will undergo surgery or those who need it for examination)

Please present your insurance card to the Admission Section of the Medical Affairs Division once a month during hospitalization.
In addition, please present your insurance card promptly when you change your insurance card or obtain a new public expense, etc.
Please note that we may ask you to reschedule your appointment if we are unable to provide you with a room due to circumstances such as emergency hospitalization.
If a patient needs to be transferred for medical reasons during hospitalization, he/she may be asked to move to a different room or ward. Please understand this in advance.
If you wish to use a private or special room, please let us know at the time of reservation (even during hospitalization). However, please note that we may not be able to meet your request depending on availability. (Please refer to the "Hospitalization Fees" section for fees.)

Please make sure to write your name on all your belongings. Please minimize the number of items you bring with you.
Bedding (sheets, pillowcases, etc.) will be washed and changed on a regular basis.
*Please refrain from bringing in televisions and other electrical appliances.
*In order to make your stay in the hospital more comfortable, other supplies are also available at the store.
Medical treatment during hospitalization
Medical treatment is led by the attending physician in cooperation with other physicians and other professionals.
The attending physician will make rounds on working days to ensure proper patient care and to explain the patient's medical condition, tests, and treatment details.
However, there may be times when he/she is not available due to study days for research or on-duty duties. In such cases, please understand that you will be examined in your place.
Please note that we may contact you regarding the schedule. If you have any questions, please feel free to ask a nurse or medical consultant.
If a patient or family member requests to wait for a consultation, treatment, or examination, we may have to ask you to wait. Please inform the nurse or medical consultant of the desired date and time.
Precautions for hospitalization
Nurse call
To call a nurse, press the nurse call button on the wall.
Phone and cell phone
Phone calls will be taken only in case of emergency.
Cell phones may be used in the hospital outside of patient rooms. Please use it in a place that does not disturb other patients.
● Television
Televisions are available upon request. A TV card is required for use. TV cards can be purchased from the vending machine on the 2nd floor. Viewing hours are from 7:00 am to 9:00 pm. Please use earphones during viewing. Please ask the nurses in each ward for the payment of the TV card.
Refrigerator
A communal refrigerator is available on each floor. Please write your name and date when you use the refrigerator.
Lights out time
9:00 p.m.
Bathing
Please follow the instructions of your doctor. For those who need assistance, we also provide bathing once a week and wiping twice a week.
Laundry
There is no laundromat in the hospital. If you wish to have your laundry washed in the hospital, please ask a staff member. 770 yen per bag (including consumption tax) will be charged.
*One bag is large enough to hold 5-7 60cm x 60cm garments. Please bring a bucket with a lid to put your laundry in if you are bringing it home.
Outings and overnight stays
Permission from the attending physician is required. Please ask the nurse for a "permission letter" to be issued.
No drinking or smoking
Alcohol and smoking are prohibited in the hospital and on the grounds.
Theft and loss
Please do not bring large amounts of cash or valuables to the ward as we are not responsible for theft or loss. We do not accept cash or valuables unless it is unavoidable. Some patients with dementia are admitted to our hospital. We ask for your understanding and cooperation in case you enter another patient's room or touch his/her personal belongings without any intention to do so.
In the event of an emergency or disaster
In the event of an emergency disaster, always follow the evacuation guidance of doctors, nurses, and hospital staff. Please confirm the evacuation route when you are admitted to the hospital.
About private cars
Please refrain from coming to the hospital by car when you are hospitalized. If you have no choice but to leave your car in the parking lot during hospitalization, please ask the receptionist on the first floor to identify it as an illegally parked car. The hospital will not be responsible for any accidents, theft, or damage that occurs during parking.
Medical Certificate
Requests for medical certificates can be made at the reception desk on the 1st floor.
Some insurance companies have prescribed forms, so please bring them with you when you apply.
Please note that it will take about two weeks to receive the order, depending on the doctor's work schedule and other factors.
Display of Name and Patient's Name
To prevent accidents, the hospital identifies patients by calling or writing their names.
As a rule, the names of patients are displayed at the entrances to the rooms and on the beds. The nameplates at the entrances to the rooms are covered to protect privacy.
Other
Please refrain from bringing in electrical appliances. If you do bring them in, the hospital will not be responsible for any damage, theft, or contamination.
As a rule, all entrances and emergency exits are locked at night from 9:00 p.m. until the next morning. Please do not leave the hospital except in case of emergency.
Other prohibited acts
In the event of any behavior that may disturb or endanger other patients, the hospital director may recommend that the patient be discharged in order to ensure the safety of other patients and the environment in which they are receiving care.
● "Gratuities" for medical treatment
Please do not worry that we will not receive any "gratuities" for our medical services.
